With the above experience we are now trying to help more people to lead a peaceful life with our services. Services Terms ShreemBrzee Home Care to provide ( Non Medical Services )that are intended to assist the clients patient with activity of daily living which includes but be limited to activities of daily living support like personal care such as bathing, dressing,& hair combing. Medication reminding, House keeping, personal laundry, cooking, shopping assistance, and companionship as desired by the client. These services will be provided for hours, Days, Months, Years as desired and requested by the client. The requested services will be provided by shreembrzee Home Care services. For a cost which shall be mutually agreed. The caretaker shall be provided by ShreemBrzee Home Care for the selected services depending on the staff availability. Any changes with regard to services selected or cancellation of the same may be intimidated by the client or his/her representative through a phone call or a written communication to the Care coordinator. Care coordinator coordination Clients are encouraged to contact the ShreemBrzee Coordinator on 9894388925 /8300238089 to communicate changes in client needs and to resolve schedule changes, concerns, complaints, hours agreed upon, changes on hours agreed upon and questions on care Provided. No communication should be done with the caretaker in this regard. In response care coordinator is responsible for supervising, hiring, training, firing and disciplining the caretaker for the said assignment assigned to him/her. Direct communication is essential to ensure high quality of care. Care coordinator will assign duties to care taker based on the understanding of patient's desires, needs and requirements. Once appointed the client can communicate directly to the caretaker their wishes and requirements but within the agreed services. any other services other than the selected services should be informed to care coordinator and service charges paid for the same, To enhance communication between client and care coordinator the care coordinator will make a visit to the clients quarterly. Services provide Live in - Patient Home Care Home Servant for Housekeeping & Cooking, Baby Care , Child Care Day Care : (8 to 10 Hours ) Night Care : ( 8 to 10 Hours ) Delivery Care : (Days) ( 8 to 10 Hours ) Hospital Hourly Care : Rs.100 p/h (Minimum 4 Hours) Client Responsibility 1. Client will be sole responsible for the safety , food and shelter of the caretaker. 2. Client should check their belongings before and while they leave.so there is no doubt of theft after their leaving. 3. Client should not disclose any important information like card pin no, Bank details etc with the caretaker or allow them to handle keys of important lockers, wardrobes, or almaras 4. Client should not allow the caretaker in rooms other than the patient room. 5 client should not discuss any matter related to ShreemBrzee Home Care whether it is with regard to payment or services. 6. The client should not allow the caretaker to leave the premises without the permission of the ShreemBrzee Home care coordinator.any such intention of the caretaker should be informed immediately. To ShreemBrzee Home care office. 7. The Agency will not be responsible of any theft or loss of things on in compliance of any of the above said responsibilities. Payment Terms A onetimenonrefundableRegistration fees cum service charge will be charged for first time service users. Payment for the services agreed upon shall be to ShreemBrzee Home Care along with the above registration cu service charge either through cash or cheque, or Bank transfer. Payment Invoice for the services undertaken shall be sent 25th of every month, payment shall be on the receipt of the invoice stable within 10 days.
